New Patient Registration

If you live within our practice area (see map below) you are welcome to register with us and our reception staff will be happy to guide you through the procedure, or you can complete the forms on our website.

 

You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Forms can be downloaded from the above "Registration forms" tab.

Please note; We also require 2 forms of ID to register patients. We will need proof of address and a photo ID, we will not be able to register you until you have completed and returned the form and provided 2 forms of ID.  Please speak to reception if you have any concerns regarding this.

New patients may be offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration.  Please contact reception for further information.
